Appointment Times

Physician Appointments:  Monday through Friday, 8 AM to 11:30 AM, 1:30 PM to 4:30 PM

Physical Therapy Appointments:  Monday though Friday, 7:30 AM to 12:30 PM, 1 PM to 6:30 PM and Saturdays 8 AM to 1:00 PM

Once you have made your initial appointment, you can pre-register by clicking on the links for New Patient Forms below. Our interactive forms are secure to protect your privacy. Simply complete the information and submit. As an alternative, you may download, print, and complete the forms prior to your arrival. This will help to speed up the new patient registration process and get you in to see the doctor faster!

All new patients are required to fill out the following forms: (you may choose to use the interactive forms or download and print the same version)

Downloadable Forms:

All new patients of Boulder Orthopedics & Boulder Orthopedics Physical Therapy and Sports Medicine are required to fill out the following forms:

Patients scheduled with Dr. Rector to discuss the BHR are required to fill out these additional forms:

• Birmingham Hip Resurfacing Arthroplasty Consent Form
• Harris Hip Score
• UCLA Activity Score

New patients scheduled to see Dr. Stilp are additionally required to fill out this form;

If you register online, all of your information is transmitted securely and is held in strictest confidence, adhering to HIPAA Guidelines and protecting your privacy. You will need to print out a copy of the Privacy Policy, sign it, and bring it to your office visit.
